CIUEM is the national registry for .mz — not a commercial competitor, but the institution that runs the extension. You can deal with them directly, and many Mozambican companies do. What changes is the work left with you.
Both are local providers, both sell hosting and domains. The short answer: on the basics they are similar, and the choice comes down to three or four details. Let us go through them one by one.

Where CIUEM is strong
It is the official source. If you only want to reserve the name, have someone handling DNS and do not mind the administrative pace, going direct is perfectly valid.
